Peritoneumska dijalizaIn the period from November 26th to November 28th, 2014, an education course on peritoneal dialysis was held in the Internal Medicine Clinic of the University Hospital Clinical Centre Banja Luka. The aim is increasing the level of services to patients who undergo peritoneal dialysis treatment by conducting the continuing education and by exchanging the experiences in the field of nephrology and peritoneal dialysis.

The education course officially started on Wednesday, November 26th, at 10:00 a.m. by official reception organized by Acting General Manager Professor Mirko Stanetic, MD, PhD, who wished successful work to all attendees.
Peritoneal dialysis is one of the methods of treatment of patients with chronic kidney insufficiency. In this kind of dialysis, peritoneum serves as a filter through which toxic substances are removed from organism by the processes of osmosis and diffusion. Dialysis fluid is injected in abdomen via catheter, it stays there for a certain period of time and afterwards it is poured out. It is performed during the night while the patient is sleeping or during the day. In comparison to haemodialysis, peritoneal dialysis is more affordable and it is conducted in home conditions.

Besides physicians, nurses and medical technicians from the Internal Medicine Clinic of UH CC BL, participants of the education course were also physicians, nurses and medical technicians of the University Clinical Centre Tuzla, Public Health Institution University Hospital Foca and the General Hospital Zenica. The moderator was Associate Professor Vlastimir Vlatkovic, MD PhD, internal medicine specialist – nephrologist at the Internal Medicine Clinic of the UH CC BL. As we were informed during discussion with participants of the education course, all participants were very pleased with the level of acquired knowledge, organization of education course, as well as with hosts who made efforts to provide the participants with a pleasant atmosphere during the three-day stay in our institution and in Banja Luka.

The education course was organized with the support of Co.Medprom Banja Luka, a company which is the representative of Baxter Company – manufacturer of medical materials for peritoneal dialysis from Switzerland.

Peritoneal dialysis programme started on December 12th, 2009 by performing the implantation of peritoneal dialysis catheters in three patients who had been treated by haemodialysis until then and whose possibilities for creating the permanent blood access for haemodialysis were exhausted. After that, patients were educated and enabled for independent treatment in home conditions. The treatment continued in that manner and it was accompanied by regular check-up examinations which were conducted on a monthly basis.  Regular activities included pre-dialysis education of patients and members of the families. This education is an integral part of integrated approach to treatment of patients with terminal kidney insufficiency and it offers the possibility of choice of therapeutic procedure to patients. On May 4th, 2010, in the Urology Clinic, implantation of peritoneal dialysis catheter was independently performed for the first time, without assistance of visiting surgeons. The procedure was performed in a patient who previously underwent pre-dialysis educational programme and who was simultaneously prepared for the kidney transplantation.

Simultaneously with implementation of the therapeutic procedure and education of patients, the organizational structure was developed. It implied the establishment of the Protocol for clinical monitoring and treatment of these patients. The organizational structure covers all procedures that are being used, including analysing of transport mechanisms of peritoneal membrane, providing the information to laboratory regarding necessary tests and examinations that have not been conducted so far. „PD-Adequest“ softer was installed. It is used in all developed centres for peritoneal dialysis and it provides analysis and planning of peritoneal dialysis programme and creating the data base. Also, a system for supplying the patients with consumables for peritoneal dialysis was established in cooperation with the Health Insurance Fund of the Republic of Srpska and certified suppliers. Recombinant human erythropoietin beta with longer activity – MIRCERA was included in regular therapy, which made us the first centre that conducted this type of treatment.

With actions conducted so far, further functioning and developing of this activity have been provided in the way in which they are organized in all developed clinical centres worldwide. Accordingly, preconditions for further increase of number of patients that will use peritoneal dialysis in accordance with therapeutic indications have been created.
